New Patient Registration

Important Note:

We are based in Chepstow, South Wales. We have nothing to do with Vauxhall in London - please do not register if you are in London. 


If you live in our practice area and would like to register with us, please complete one of our registration forms and a health questionnaire. These are available from our receptionists at both surgeries or can be downloaded from the above "Registering Online" tab.

On the form you will be able to say which practitioner you would prefer to see. We will record that preference, however, you will be registering with the practice rather than an individual GP.

When you register with the practice you will be required to have a new patient check with the doctor if you are on medication.  This will be an opportunity to discuss any health problems you may have and help us to provide continuity of care.  Your medical records may take some time to be transferred from your old practice.

In the case new babies being registered, please can you arrange to register your baby with the practice at least a week before attending for the first baby clinic.

If you are looking to move to this practice from another local practice, we prefer only to register patients where there has been a breakdown in the relationship with their previous GP.

Medical treatment is available from the date of registration.  Please contact reception for further information.
